If you would like to schedule a new patient appointment
with Insight Family Health Center - Image,
please submit the following form to the office:



New Patient Scheduling Form >>>

The nurse practitioner that you wish to see will review this form
and you may be called to schedule an appointment. If you have not
heard back from the office within one week, please call the office
at (910)763-1960 to make sure we received your form.

After you have scheduled a New Patient Appointment
please print the following forms, complete them in their entirety
and bring them to our office 15 minutes before your scheduled
appointment time. In addition, please bring a current copy of your
insurance card and driver's license to the visit.

PLEASE DO NOT WAIT UNTIL THE LAST MINUTE
TO FILL OUT THIS PAPERWORK.

It takes 3-4 days to complete all of these papers. You MUST have them
all completed before your scheduled appointment or you may be rescheduled.

IF YOU DO NOT ARRIVE 15 MINUTES EARLY FOR YOUR
SCHEDULED APPOINTMENT, YOU MAY BE RESCHEDULED.


We will call you one week in advance of your appointment to confirm
that you will be keeping it. You will have 48 hours to call and confirm
your appointment.
IF YOU DO NOT CALL TO CONFIRM YOUR APPOINTMENT
ONE WEEK BEFORE, YOUR APPOINTMENT WILL BE
CANCELLED AND GIVEN TO ANOTHER PATIENT.
